We deliver this program in in a personalized manner to support participants living independently. Services are based on the participant’s needs and preferences as documented in the person-centered plan (PCP).

Our Goals

  • Our goal is to enable the participants:

    • Live in a home of his or her choice located where he or she wants to live, and
    • Live with other participants or participants of his or her choosing (not including relatives, legal guardians, or legally responsible persons.
