Rationale

We deliver this program in in a personalized manner to support participants living independently. Services are based on the participant's needs and preferences as documented in the person-centered plan (PCP).

Our Goals

Our goal is to enable the participants:

Live in a home of his or her choice located where he or she wants to live, and
Live with other participants or participants of his or her choosing (not including relatives, legal guardians, or legally responsible persons.
